• Cataracta Nigra - Standing Phacoemulsification Description: In this video presented By Prof.Yehia Salah Mostafa, Cairo University. A 95 years old patient with kyphosis and black cataract that needed to be operated.The only possible comfortable position for the patient meant I have to work standing which was the first difficulty. The second difficulty is dealing with the black cataract. I use my technique which I call two Nuclei concept. Where I deal with the thick nucleus as two The superficial half is debulked with bevel down. Then the deeper half is dealt with with bevel tip up. The main trick is to make a wide crater so you can go as deep as you want shaving the nuclear fibers. I add the whole drilling in such very hard nuclei, to weaken the nuclear crater facilitating later cracking. An important remark is always have good exposure of the phaco tip. The concept is to use least energy away from the cornea and this is facilitated as well with the vertical chopping technique used.
