• Late Capsular Block Syndrome Description: This a case presenting 10 years after successful phacoemusification with diminutuin of vision to 0.3 BCVA. On examination there was a dim red reflex with clear cornea and normal fundus and IOP. The differential diagnosis was:1-Opacified IOL. 2- Posterior capsule opacification. 3- Capsular distention. The 3 rd diagnosis was confirmed with meticulous slit lamp exaination and confirmed with Scheimpflug image. The decision for treatment was to reopen the bag and do I/A of the entrapped fluid behind the IOL rather than YAG laser. The result was amazing with regaining of 20/20 vision next day post operative.
