• Modified 4 flange technique-Haptic first modification by Dr Sheetal Brar Description: This trainee case had an intraoperative PCR and nucleus drop during SICS surgery, which was referred to me for secondary IOL fixation. The video demonstrates an alternate way of performing the 4-flanged technique of SFIOL ( originally described by Dr Sergio Canabrava ), in which the right suture is passed through the leading haptic eyelet, cauterized and inserted behind the pupil first. Followed by this, the second (left) suture is grasped behind the IOL , exteriorized , and cauterized through the trailing haptic eyelet. This method may be especially useful for beginners as it avoids creation of a "cross" at the wound and thus avoids the confusion, as to which side suture is to be taken into which eyelet and also prevents crossing over of the prolene sutures, which can sometimes get looped or entangled within in the AC, thus increasing the intraocular manipulations.
